Indonesia Health Alert Card, more commonly known as an Indonesian Health Card, is a simple, easy-to-make card that contains important information like your name, your signature, contact information, your yearly salary or monthly wage, physician's name, etc.. It's also commonly known as health card. It may be used to file a claim for certain medical services and commodities from some local hospital and pharmacies. It serves as proof of identification in cases of emergency or disaster, and can also be utilized as a means of traveling identification when traveling outside the nation. It is important to be prepared with this health record in any way times.

Most Indonesian hospitals and hospitals have their own websites where you may fill out and submit the online wellness card application.
